Overview

The Health Information Management Specialist is responsible for assisting and supporting various functions of our clinic operations that includes collecting, preparing and entering patient health information into Electronic Medical Records. Duties include processing patient ROI's, faxing and scanning patient records, maintaining clinic forms and filing. 

Responsibilities

  • Maintains patient health records according to AVP guidelines
  • Processes all requests for release of information- ROI
  • Ensures patient information is properly scanned and indexed into the EHR per workflow
  • Ensures all forms in the clinic are up to date and available for clinical use
  • Ensures confidentiality of release of information per HIPAA and AVP policies
  • Imports and attaches patient electronic files into EHR per workflow
  • Processes incoming and outgoing faxes
  • Researches, and corrects discrepancies in patient data (Name, DOB, ID#, INS)
  • Assists care team in sending our patient letters
  • Redacts PHI from documents (i.e- incident reports) prior to distribution out of AVP Network
  • Conducts minor chart audits of patient records to ensure completeness and compliance
  • Acts as liaison between patients, providers, staff members and RCM to obtain old records or other documentation from records storage not available in EHR.
  • Answers phone calls and respond to messages related to patient records
  • Interacts with patients, staff and other healthcare providers in a friendly and courteous manner.
  • Performs any and all duties as assigned such as but not limited to verification of eligibility, processing a referral etc.

Qualifications

  • High School Diploma or GED required
  • Detail oriented, reliable, and able to multi-task in a fast-paced, high-volume work environment
  • Excellent verbal and written communication skills
  • Medical Front Desk experience preferred
  • Flexible schedule
